  • Applications are listed under 'Useful resources'

See VCAT—Guardianship and powers of attorney(opens in a new window).

To download the medical report form (that needs to be filled in by a doctor) see VCAT—Guardianship and powers of attorney—Medical report template(opens in a new window).

Reassessment or revocation guide

See VCAT—Guide to revoke or reassess a guardianship or administration order(opens in a new window).

Applying for reassessment

There is no form to fill in to apply for a reassessment of their guardianship or administration order. An applicant just has to write a letter asking for a reassessment and explaining why they think it is necessary. The VLA website has a sample letter to download and complete.

See Changing or cancelling an order(opens in a new window).
